For Patients:
The Cancer Transitions: Moving Beyond Treatment® program is for any cancer survivor who has completed treatment within the last two years. It addresses many of the physical, social, and practical issues survivors may face as they transition into post-treatment cancer survivorship. This program is designed to improve quality of life by focusing on lifestyle skills and prosocial support.

Cancer Transitions for Caregivers
Begins Thursday, May 11 | 12pm-1:30pm | Virtual
For Caregivers:
The Cancer Transitions Caregiver is for cancer caregivers of someone who has completed treatment within the last 2 years. The program will address the needs of cancer caregivers during the transition into post-treatment survivorship. It is designed to center the caregiver and help them re-focus on themselves and their needs. We still have a few spots open!
